HIV SERVICES Community Health Worker ( HIV Quality Assurance-Clinical Care Coordinator: HIV program providing a full range of services seeks an experienced QA-Clinical Care Coordinator responsible for coordination of quality management activities and data gathering, analysis and reporting. Works closely with HIV providers and support staff to assure quality of care to HIV infected and affected individuals in keeping with the Patient Centered Medical Home model of care. The Coordinator is responsible to assure compliance with clinical and quality management guidelines for all HIV programs. Responsible to develop Calendar/Workplan of quality monitoring activities for all grant funded HIV programs; prepares/presents reports to internal/external groups; responsible for clinical care coordination to ensure continuity of care across continuum of Health Center Services; coordinates weekly medical case conference meetings and works collaboratively with HIV Medical Director, Program Director and HIV services staff. Qualifications: R.N. with valid NYS license. Minimum two (2) to three (3) years experience in an acute and/or primary care setting; demonstrated understanding of the HIV disease management process; demonstrated presentation skills to small/large groups of diverse healthcare background/experience; experienced data collection/analysis and in implementing quality improvement projects; demonstrated understanding of cultural competency; familiarity with computers, MS Windows/Office software. MEDICAL Supervisor Medical Social Work and Perinatal Services: Provides mental health services to patients through psychosocial assessment, counseling, crisis intervention and referral services, taking into consideration specific physical, cultural and age related needs of the population served. Works collaboratively with psychiatrists and medical providers regarding biopsychosocial components of illness and disability. As supervisor of perinatal services, provides direct supervision to medical case managers and other staff as determined by the WMY administration. Also provides clinical supervision to master’s level clinicians. Min Quals: Master’s Degree in Social Work with LCSW from New York State; three (3) years work experience in Social Work practice and program development, preferably in a medical setting; Knowledge of community resources; Demonstrated ability to work with a culturally diverse population of patients; Familiarity with NYS Child Protective Service laws, Pref. Quals: WIC WIC Breastfeeding Peer Counselor Supervisor (PT): Supervises and manages the WIC breastfeeding peer-counseling program. Responsible for training, mentoring providing follow up and guidance to the peer counselors. Min. Quals: Personal experience as a breastfeeding mother. One year supervisory experience. Ability to communicate effectively and provide excellent customer service. Should possess credentials or pending credentials of an International Board Certified Lactation Consultant (IBCLC) or have other certifications in lactation management (MMTP Registered Nurse (PT) – 12 hours/month: NYS license with behavioral health experience a must. Assessments and triaging patients, toxicology screens and substance abuse or mental health exposure preferred. SOCIAL WORK Licensed Master Social Worker: Facilitates the continuity of care through psychosocial assessment and diagnosis, counseling, crisis intervention and referral services, taking into consideration specific physical, cultural, and age related needs in the CDC HIV PREVENTION PROGRAM Project Manager-: The Project Manager is responsible for the delivery and oversight of the organization’s HIV prevention projects. This position must have a strong background in project management, human resources, and federal reporting requirements. Responsibilities: Responsible for collection, entering, analyzing and use of all standardized program monitoring data and program performance indicators; directly supervises program staff; develop written prevention and educational materials; interfaces with internal and external community resources and funding agencies; manages budget and expenditures; Minimum Qualifications: BA/BS in public health, education, social work, or a health related field with 3 to 5 years experience in community outreach experience; knowledge of HIV Aids and other sexually transmitted diseases; must have excellent communication skills; ability to address groups; Computer skills to include: MS Word, PowerPoint, Excel. WIC WIC Breastfeeding Peer Counselor Supervisor (Part Time): Supervises and manages the WIC breastfeeding peer-counseling program. Responsible for training, mentoring providing follow up and guidance to the peer counselors. Min. Quals: Personal experience as a breastfeeding mother. One year supervisory experience.
