We look forward to assisting you with your ongoing health care needs. If order to provide you with timely service, please review the following information below.

Please show up at least 15 minutes prior to your scheduled appointment in order to allow time for registering. Bring your insurance card to every appointment (including Medicare and Medicaid cards). If you do not have insurance, please bring proof of income to qualify for our sliding fee scale or to find out if you are eligible for Medicaid, Health Plus or Child Health Plus.

If you need a prescription refill and receive your medication through the on-site Price Chopper HouseCalls Pharmacy, please call 518-427-0102 and they will contact your provider for a new prescription if needed.

If you do not currently receive your medication through the on-site Price Chopper HouseCalls, please call HouseCalls at 518-427-0102 and they will assist you in moving your prescription here from another pharmacy.

If you need to cancel your appointment, please contact us as soon as soon as possible. If you call during regular business hours we will work with you to reschedule your appointment.

Request Medical Records for Self or to Transfer
In the event that you need to have your medical records transferred to another doctor or hospital, please click here for RECORD RELEASE FORM. Once completed, please send form to Health Information  and we will gladly transfer your medical records for you.  You may also fax the completed form to us at 518-432-3632
